improve the health status of Sandusky County residents.

In 1993, Health Partners was formed with a group of Health Organization
CEO’s in Sandusky County. The focus of Health Partners was to determine
health needs and to work cooperatively to address those needs. Initial
members included the Health Department, Hospitals, Community Health
Services, St. Francis Rehabilitation Center, and a Physician. Later, the
Mental Health and Recovery Services Board, Firelands Counseling and
Recovery Services, and the Family and Children First Council also joined
to more fully consider behavioral health care needs.
In order to assure we have the best available data, comprehensive health
assessments were conducted in the years 2001 and 2005. The Health
Department initiated the first assessment with financial support from
all of the partners. Health Partners has now made a commitment to
following the Community Health Improvement Cycle model, with Health
Assessment a primary focus of this group. Collaboration was essential in
order to incorporate the needs of participating agencies for specific
data. Secondary data was compiled prior to primary data with comparisons
to state and national data. The primary data was gathered by a
professional marketing agency using standardized data sets for youth and
adults, in 2001. A more scientific approach, utilizing a random mail
process was employed in 2005. The Healthy Communities Foundation of the
Hospital Council of Northwest Ohio was contracted with to gather,
analyze and report on the primary data in 2005. All participating
agencies agreed that the use of standardized data sets was critical in
order for us to benchmark with surrounding counties in the state and
throughout the nation. Main areas of concern identified from the
assessments include: alcohol usage, tobacco usage, teen pregnancy,
depression and suicide, weight control and diabetes, and health care
coverage. Health Partner’s current project is gathering information on
available county resources to coincide with each of the issues addressed
within the 2005 Health Assessment.
